Community Health Assessment Survey

In an effort to strengthen the health and wellness of the Leech Lake Reservation community, leaders and organizations formed a collaborative team to perform a community health assessment (CHA).

Community Health Assessment includes collection and sharing of information to describe overall health and wellness.The Leech Lake Reservation Community Health Assessment will help:

• identify health priorities for Native Americans living on or near the Leech Lake Reservation,
• use data to identify health issues, gaps in information, and prioritization of improvement opportunities
• provide data and statistics for community needs such as future programming and funding opportunities

These CHA efforts will not be successful without your involvement and voice. The Leech Lake Reservation CHA team looks forward to your participation and feedback!
